Vaccination Reporting Tool

Re: Vaccination Reporting Tool

In addition to directing members of the community to vaccination opportunities available at public administration sites, the Vaccine Working Group continues to plan for dedicated clinics for campus community members. To that end, the Institute has added a vaccine attestation tool to the Caltech COVID-19 Reporting Application, which will help us assess continuing need and interest in receiving the COVID-19 vaccine. We ask all members of the community, including students, staff, postdocs, and faculty, to use this tool at their earliest convenience to record their vaccination status.

Using the Tool

  • Log into the COVID-19 Reporting app using your access.caltech credentials.
  • At the top is a green box labeled "My COVID-19 Vaccination Status" with three options listed to the right.
    • If you have not yet been vaccinated or made an appointment for vaccination elsewhere: select either "I am interested in being vaccinated at Caltech" or "No plans at this time."
    • If you have been vaccinated: select "I have been vaccinated."
      • You will be prompted from drop-down menus to indicate which vaccine you received and the date(s) on which you received it.
      • If you have received only the first of two doses (Pfizer or Moderna), select the date for "Dose 1" and leave "Dose 2" blank for now. You can return to the tool to update your record after you receive your second dose.
  • You can change your vaccination status at any time by logging into the COVID-19 Reporting Application and clicking the "Change" button on the far-right side of the "My COVID-19 Vaccination Status" box.

This information is being gathered to help the Institute plan its vaccine distribution efforts. Please note that while members of the community are asked to use the tool to inform the Institute's planning, reporting your vaccination status or intention regarding vaccinations is not mandatory.

Caltech strongly encourages all eligible members of the community to receive a vaccination at their earliest point of eligibility.

Vaccinations are a critical, life-saving tool which must be paired with continued compliance with the Institute's face covering, physical distancing, and surveillance testing requirements. At this time, surveillance testing will continue for all individuals reporting to campus on a regular or one-off basis, whether or not they are vaccinated.

The Institute expects eligible members of the community seeking access to the vaccine to do so in accordance with all local and state public health guidelines and processes and in a responsible, fair, and accurate manner.
